12-2005 Articles of Amendment (+ Attachment) - Filed on 12-27-2005 by Matthew Lewis (President) to the Colorado Secretary of State. Ammends the original Articles of Incorporation to include a statement of exempt purposes, required to apply for federal 501(c)3 status.
Status Documents
IRS 501(c)3 and Public Charity Determination Letter - This document, issued on 03-30-2006, recognizes the International Cystinuria Foundation as exempt from Federal income tax under section 501(c)3 of the Internal Revenue Code and further classifies the Foundation as a public charity, (retro)effective as of 08-08-2005.
Colorado State Certificate of Good Standing - A current Certificate of Good Standing, issued by the Colorado Secretary of State.
